Forest Haven Nursing & Rehabilitation Center, LLC. Is conveniently located in a quiet residential area of Jonesboro just minutes away from hospital services, doctors’ offices and the offices of other health professionals. The warm and friendly atmosphere experienced here reflects the long tradition of quality care that was begun long ago by Mrs. Myra Carter … a tradition based upon our belief that what counts most in a nursing facility is resident care.
At Forest Haven we know that quality resident care is no accident. It is the result of a team of professionals working together and committed to providing the very best in quality care with a goal of constant improvement. Our facility is one of Louisiana’s most innovative leaders in quality long term care and often exceeds the expectations one might have of a nursing facility. Forest Haven is continually working to upgrade and improve its services. We are the first facility in our area to have an in-house nurse practitioner and we lead our profession in the use of electronic medical records.
Amenities include carpeted private and semi-private suites, arts and crafts center, a beautiful in-house chapel, in-house rehabilitation and gym, beautifully landscaped grounds with walking paths, and multiple day rooms for a quiet afternoon show, a lively LSU ballgame or an evening of cards or dominoes.
Our rehabilitation programs, including physical, speech and occupational therapy, officer our residents the opportunity of returning to their greatest degree of independence, with many able to return to their own homes. At Forest Haven, we are committed to Improving Quality of Life.

This year, we proudly celebrate Steve Williams, RN, DON for 30 years of dedicated service with Central Management Company; a journey that began at Forest Haven in 1996 and has helped shape who we are today. Steve first joined Forest Haven as an LPN, later transferring to another CMCO facility where he broadened his clinical skills and earned his RN before returning “home” in 2007 as Assistant Director of Nursing. In 2019, he was promoted to Director of Nursing, and since that time he has been a steady, values‑driven leader, aligning our nursing services with Central Management’s commitment to Improving Quality of Life and with the resident‑centered, high‑performance systems.
Through his calm presence, strong clinical judgment, and genuine love for our residents and staff, Steve has helped foster a culture of teamwork, accountability, and continuous improvement; one in which people feel supported, heard, and called to do their very best every day. Forest Haven’s story of excellence is inseparable from Steve’s story; his faith, integrity, and unwavering commitment have touched countless lives and continue to inspire the next generation of caregivers who choose to build their careers here.
